
LONG ISLAND BODY CONTOURING PROCEDURE INFORMATION
"MOMMY MAKE-OVER"
These procedures can be grouped together. Pregnancy, weight fluctuations and aging will cause changes in our bodies. These procedures are often grouped together in a "Mommy make-over procedure" as child birth causes dramatic changes on our bodies appearance.
If you are considering a body contouring procedure there are some points you should keep in mind.
►Ideally a patient having a body contouring procedure should be at their ideal body weight. If you have surgery then lose weight afterwards the skin will loosen and sag again.
►If you are planning on having more children you should hold off on the surgery as your body will change with another pregnancy.
►Smoking increases the risk of poor wound healing, wound infection and wound breakdown.
►Plan on having assistance at home for 2-3 days after surgery.
►Stock up on meals prior to your procedure.
►Make sure you have an organized recovery area with everything at your finger tips such as fluids, phone, computer and books.
Being prepared for your cosmetic procedure will cause less stress as you are healing.
Abdominoplasty- Also known as a tummy tuck will remove excess skin on the lower abdomen, tighten the stomach and remove stretch marks below the belly button. The abdominal pouch that is often complained about is usually caused from stretched out skin, weight gain, and separation of the abdominal muscles. Removal of the extra skin and fat will not tighten the protrusion, repair of the diastasis or separated muscles is needed to bring the abdominal wall muscles together. I often combine an abdominoplasty with liposuction of the flank area to allow for better contouring.

Brachioplasty-In this procedure skin and fat are removed from the arms. Liposuction can be performed simulataneously to remove the excess fat on the arms. Incisions are made to remove the excess skin. The incision can be a short scar which lies with in the arm pit and is used in mild cases of excess skin. A longer incision is used for moderate to severe cases of excess skin and this will extend down to the elbow.
Breast Augmentation- An implant, saline or silicone is used to increase the size of the breast. There are three different types of incisions that are used to place the implant. Periareolar, inframammary and placement through a distal location, These distal locations include the arm pit or the belly button. The implant can be placed above or below the muscle. Type of implant, incisions and placement are a matter of each individual patient and surgeon preference.
Silicone Breast implants have returned to the market. They offer a more natural feel then traditional saline breast implants. They come in a variety of shapes and styles. I prefer the moderate plus profile as it resembles a natural tear shaped breast.
Breast augmentation is one of the most common procedures in plastic surgery. Woman are generally pleased with their results which help improve they way that they fell about themselves. The procedure is short, lasting about an hour and a half. The procedure is done in an outpatient setting. Expect discomfort initially but recovery is generally rapid allowing most patients to return to work the following day.
Breast Lift-Child birth, breast feeding, and fluctuations in weight all will affect thr appearance of breasts. Patients typically complain of droopiness and a loss of breast shape that goes along with these changes. A Mastopexy, otherwise known as a mastopexy will lift the breast and nipple areolar complex as well as reshape the breast giving a more firmer, youthful look.
►A breast lift will not increase volume. If you are looking to have your breast size increased you will need an implant in addition to your lift, to add volume to your breast.
The amount of ptosis or droopiness will determine what incision is used to lift the breast.
►Doughnut mastopexy is used for mild cases of ptosis. The incision as the name implies is circular, around the areolar. This will help tighten the loose skin but does little to actually lift the breast.
►Vertical mastopexy or a lollipop is used in moderate cases of ptosis. An incision is placed around the areolar and down the front of the breat, below the areolar. This will lift the breast and tighten the skin.
►Anchor Incision is used in cases of severe ptosis where an extra incision is added in the breast crease to help remove excess skin.
Breast Reduction-This procedure will remove breast tissue, reshape and lift the breast giving the breast a more youthful appearance that is in better proportion with the rest of the body. Breast reductions are most commonly performed in women that have large breasts and complain of back, neck and shoulder pain.
Inverted Nipples-can be a sourse of embarrasment for a patient. Inverted nipple are usually repaired under a local ansthetic. The dermal attachments that are pulling the nipple inward are released and small flaps are dissected to help ensure there is no recurrence of the inverted nipple
Liposuction-Is a technique in which fat is removed to reshape an area of the body. Liposuction will help with small areas of fatty deposits that are stubborn to weight loss and exercise it should not be used as a quick fix to weight gain. Important considerations.
►Liposuction will not remove cellulite or loose skin.
Tumelescense is a technique where fluid, a combination of saline, lidocaine and epinephrine, is infiltrated into the area to be liposuctioned to help breakdown the fat and decrease blood loss.
Ultrasonic, Laser assisted and Power assisted liposuction are used in combination with traditional liposuction to help break up the fat and help achieve a better result.
Thigh Lift-This procedure is most commonly performed in weight loss patients that have excess skin. Skin is removed from the upper part of the thigh. An incision in the leg crease is used for mild cases. A longitudinal incision is added for moderate to sever cases.
Vaginoplasty- Patients that seek vaginal rejuvenation generally complain of extra tissue that hangs from the vagina. These patients often complain of difficulty with hygiene issues. The extra tissue can be removed which improves the appearance.
Some women are concerned about the appearance of their genital area. Some simple techniques can be performed to help alter the shape of the vagina. These topics will be broken down by area.
Clitorial hooding-removing a wedge of tissue around the clitoris will help alleviate clitorial hooding.
Labia Majora Reduction-Women complain that the labia majoria is too bulky. The Labia majora can be reduced by excising an elliptical section on each side. The tissue will come together and the scar usually lies in a well hidden area. Liposuctioning the outer perimeter will also help reduce the bulk in this area.
Expect some swelling in the area after the procedure. I ask my patients to refrain from tight clothing and the gym for three weeks after the procedure. This procedure has a high satisfaction rate as patients are typically pleased with their results.
Labia Minora Reduction. Women typically complain that the labia minora hangs lower then the labia majora which is visible can be a souce of embarrassment. The labia minora is reduced by wedging out tissue and closing the area. These scars heal well and are usually not visible.
Gynecomastia-This condition occurs when males have an excess amount of breast tissue. Cases are best treated with liposuction and removal of the extra skin if need be.

Facial Rejuvenation
Blepharoplasty- As we age the eyelids may become droopy which can lead to a worn and tired appearance. A blepharoplasty can help by removing excess fat, skin and muscle giving a more refreshed look.
Ear Lobe Repair-Having a short ear lobe, wearing heavy eorrings or having an earring that has been pulled increases the risk of a torn ear lobe. The ear lobe is repaired under a local anesthetic. If the earring hole is enlarged but not pulled through all the way this can be repaired as well as patients complain that the two earrings rest at different levels.
Face & Neck Lift-As we age most of us develop skin laxity in the jowls and neck. A facelift will tighten the loose skin in the jowls, neck and jaw line. The incision is made in the natural crease of our facial skin.
Mini facelift or the so called lunch time face lift will have a smaller scar that does not extend as high up the temple skin or around the back of the ear down the neck hair line. There is a limited dissection of skin then the SMAS layer is sutured to help tighten tissue laxity. Liposuction of the neck may be performed simultaneously if the patient has a fatty neck. This will help give better jaw line defination.
Full Facelift- Incisions start in the natural face crease in front of the ear hen extend into the hairline and around back behind the ear. The incision will extend into the hair line on the neck. This is a wider dissection but needed for those patients that have significant laxity of the skin in the lower third of the face.
Injectable Fillers-Help restore volume in ares of the face that have become depleted with tissue such as the nasolabial and marionette lines.

Lip Augmentation- A youthful lip is full and pouty, this is thought to be more sensual. Lip augmentation can e performed using injectable fillers to increase the size of the lips. Surgery can also be performed where by incisions are made in the inner lip and tissue is rotated out giving the lip a fuller appearance.
Microdermabrasion- by using micro-crystals that are vacuumed through a machine the skin is superficially exfoliated which results in a polished look.
Otoplasty-This procedure will help those patients whose ears appear to stick out. Sutures are placed in the back of the ear to pin back the ear giving a softer appearance to the ear.
Photo Rejuvenation-Is a safe way to get rid of large pores, red complexion and brown spots. Intense pulsed light rejuvenate the skin in a non-ablative fashion by delivering pulses of light to the skin. There is little downtime and the result is healthier, younger looking skin.
Rhinoplasty-Sometimes a patient may feel that their nose is too big, crooked or hanging. A rhinoplasty can help with these problems. Usually the nose is broken down to three levels. The upper nose, also known as the nasal bridge is made up of bone. Work is done at this level if there is a bump or if the nose is too wide. The bump can be removed and the nose can be made thinner by doing osteotomies. If the bump is large then usually the nasal bones will need to be reset because a space may develop in between the two bones as the bump is removed.
The middle of the nose is made up of cartilage, it is referred to as the upper lateral cartilages. Cartilage will have to be removed if there is a bump in this section of the nose. If the nose is crooked then the cartilage can be made straight or extra cartilage can be placed to help camouflage the crookedness of the nose. A good history and internal examination will reveal if there are any obstructions in the nasal passages. These obstructions can be fixed at the same time as the rhinoplasty.
The lower portion of the nose, the nasal tip, is also referred to as the lower lateral cartilages is made up of cartilage. The tip can be made smaller by removing cartilage and suturing the tip so that is is narrower. If the nasal base is wide then it can be thinned by making the nostrils smaller, skin is removed between the lip and nose. The scars usually look good providing they are placed in the right spot. The nasal tip can also be built up if need be, with cartilage, if it does not have enough projection.
